Samsung Galaxy Y "SD card blank or has unsupported filesystem"

I was listening to the radio on my phone and the next thing I see is a message saying "SD card blank or has unsupported filesystem". How did that happen and how can I stop it happening again?
All I was doing was listening to the radio. I go to turn it off and my phone freezes for a few seconds and then I see that message.
Can anyone help and is there anyway to get what was on my SD card off it?

hmm...i think ur sdcard is damage....if it isnt then i dont think there is much chance to revive its contents........format it in ext3 filesystem.......

I have a Note 2. It frequently tells me the SD card is removed unexpectedly, and is preparing the card. Only takes a second, and then is normal again. Anyone know what is happening here too?
