Nassir Ghaemi, director of the Mood Disorders Program at Tufts Medical Center in Boston, Massachusetts, has felt shackled by the Diagnostic and Statistical Manual of Mental Disorders (DSM), often called the bible of psychiatry. Some of his depressed patients occasionally show manic behaviour but do not fulfil the DSM’s criteria for a diagnosis of bipolar disorder. Ghaemi is interested in whether such patients might respond better to drugs for bipolar disorder than for depression. But his colleagues warned him against straying from the DSM when he applied for funding at the US National Institute of Mental Health (NIMH), because peer reviewers tended to insist on research that hewed to DSM categories. Ghaemi held off from applying.

If NIMH director Thomas Insel has his way, Ghaemi and other mental-health researchers will no longer feel the weight of the DSM. “NIMH will be re-orienting its research away from DSM categories,” Insel wrote in a blog entry on 29 April. The latest edition, the DSM-5, will be unveiled on 22 May at the annual meeting of the American Psychiatric Association in San Francisco, California. Like many psychiatrists, Insel questions whether the DSM’s categories accurately reflect the way the brain works. He is pushing a project that aims to create a new framework that classifies mental-health disorders according to their biological roots. “Going forward, we will be supporting research projects that look across current categories — or sub-divide current categories — to begin to develop a better system,” Insel wrote.

The blog post made waves in the media and rattled some psychiatric clinicians and researchers. But Insel says that he has been talking about the issue since 2008. “The word was just still not out there,” he says. Insel says that he has increasingly received complaints from grant applicants who have tried to follow his guidance, only to be shot down by peer reviewers for eschewing DSM scripture.

Insel’s main concern is that DSM diagnoses are not based purely on science, but also take social, legal and historical factors into account. For example, Ghaemi says that the manual encourages diagnoses that can be treated with drugs thought to be safe, while discouraging diagnoses whose treatments carry heavy or unwanted side effects. Steven Hyman, a former NIMH director now at the Broad Institute in Cambridge, Massachusetts, blames DSM diagnoses for hampering research into the genetic basis for psychiatric illness. It is “a fool’s errand”, he says, to try to find a biomarker for a diagnosis with little basis in nature. “Such efforts have wasted human capital and [US National Institutes of Health] and industry funds for many years.”

The NIMH has been developing its new framework, called the Research Domain Criteria (RDoC), since 2009. The system replaces DSM diagnoses with broad research categories based on basic cognitive, behavioural and neural mechanisms. For example, the domain ‘negative valence’ includes categories such as fear, anxiety and loss. The framework calls for each category to be assessed on the basis of genetics, neural circuitry and behaviours, among others. “RDoC is not a diagnostic system,” says Charles Sanislow, a professor of psychiatry at Wesleyan University in Middletown, Connecticut, and a member of the NIMH RDoC working group. Instead, he says, it is a framework to guide research questions, organize findings and relate them to one another and psychiatric symptoms.

But even its staunchest supporters say that the RDoC is not a perfect solution. Ghaemi worries that its focus on biological mechanisms will underemphasize research on clinical symptoms. “The fact is, we have not yet tried to make a scientific diagnostic system based on clinical research,” he says, noting that the the DSM was never based strictly on research.

And some researchers aren’t sure how their work will fit in to the RDoC. Jane Costello, an epidemiologist at Duke University in Durham, North Carolina, studies irritability in children — a condition that crosses DSM categories such as paediatric depression, conduct disorder and anxiety. But she says that there is no place for her research among the current RDoC categories and that the framework leaves out “huge areas of psychopathology”.

Hyman has urged the NIMH to start thinking now about how the biomarkers that arise from RDoC research can be incorporated into the clinic, where the DSM's symptom-based approach will still reign supreme. “It would be very problematic for the research and clinical enterprises to wake up in a decade to a yawning gulf,” he says.

For now, Insel is uncertain whether his blog post will have a big impact on the scientific-review process at the NIMH. He notes that grant review at the National Institutes of Health, of which NIMH is part, is centralized, and individual institutes cannot directly address reviewers. As another solution, Insel is considering shifting funds away from broader research programmes and into dedicated RDoC grants. Last year, the NIMH, with a budget of just under US$1.5 billion, awarded seven such grants, totaling $5 million.

But Ghaemi says that it may take more than money before the RDoC gain currency. “It is very hard for people who have been following the DSM their entire professional lives to suddenly give it up,” he says. “It’s uncomfortable for a lot of people, but the field needs to be pushed — hard.”

A dozen Los Angeles-area residents—including the state's second-largest biller for chiropractic services—were taken into custody Tuesday in connection with seven criminal cases alleging they cumulatively submitted more than $22 million in false billings to Medicare.

Those arrested also include a physician's assistant and owners of durable medical equipment and ambulance companies, according to the U.S. Attorney's Office.

In one of the cases, three defendants affiliated with Gardena-based ProMed Medical Transportation, an ambulance company, were charged with submitting more than $5.9 million in false claims to Medicare between 2008 and 2011.

ProMed's owner, Yaroslav Proshak, 45, of Valley Village, general manager Sharetta Wallace, 35, of Inglewood, and office manager and biller Sergey Mumjian, 40, of West Hollywood submitted claims for medically unnecessary transportation services and then created fake documentation purporting to support those claims, the government alleges.

The charges filed in Los Angeles federal court are part of a nationwide crackdown by Medicare Fraud Strike Force operations in eight cities that led to charges against 89 people for their alleged participation in schemes to collectively submit about $223 million in phony claims to Medicare, federal prosecutors said.

The 12 Southland arrestees are among 13 defendants charged in Los Angeles in cases that allege health care fraud. The dozen either were arrested or surrendered to authorities after learning that they had been charged. The 13th defendant is a fugitive.

The defendants include Dr. Houshang Pavehzadeh, of the Sylmar Physician Medical Group, who allegedly billed Medicare more than $1.7 million for chiropractic treatments he never performed.

During the scheme, which ran from 2005 through 2012, Pavehzadeh, 40, of Agoura Hills, became the second-largest Medicare biller in California for chiropractic services—even though he was not in the United States when some of the services were performed, prosecutors allege.

In addition to being charged with health care fraud, Pavehzadeh is charged with aggravated identity theft related to Medicare beneficiaries whose information he used to bill Medicare as a part of the alleged scheme.

When investigators tried to conduct an audit of Pavehzadeh's claims, he falsely reported to the Los Angeles Police Department that he had been carjacked and that patient files requested by the auditors had been stolen from his car, according to federal prosecutors.

Nine defendants affiliated with durable medical equipment companies were also charged in five separate indictments.

Olufunke Fadojutimi, 41, of Carson, a registered nurse; Ayodeji Temitayo Fatunmbi, 41, formerly of Carson, and now believed to be living in Nigeria; and Maritza Velazquez, 40, of Las Vegas, were charged with health care fraud.

The scheme allegedly revolved around Lutemi Medical Supplies, a company Fadojutimi owned and where Fatunmbi and Velazquez worked. According to the indictment in the case, Lutemi billed Medicare more than $8.3 million in claims, primarily for medically unnecessary power wheelchairs.

Fadojutimi and Fatunmbi allegedly laundered Medicare funds in order to purchase fraudulent prescriptions for the power wheelchairs and pay illegal kickbacks to recruit Medicare beneficiaries.

Fadojutimi and Velazquez was arrested today and Fatunmbi is currently a fugitive, according to the U.S. Attorney's Office.

Susanna Artsruni, 45, of North Hollywood, and Erasmus Kotey, 76, of Montebello, a licensed physician's assistant, allegedly worked together to commit health care fraud out of a medical clinic on Vermont Avenue where they both worked.

Kotey allegedly prescribed medically unnecessary equipment, including power wheelchairs, for Medicare beneficiaries. Many of those power wheelchair prescriptions were then used by Artsruni's company, Mid-Valley Medical Supply, to support fraudulent claims to Medicare, prosecutors allege.

In only four months, the clinic and Mid-Valley billed Medicare more than $525,000 for the bogus claims, prosecutors allege.

Artsruni was previously convicted of health care fraud and was on pretrial supervision at the time she allegedly laundered some of the proceeds of the latest fraud.

Three other medical equipment cases were also brought, alleging fraudulent Medicare billing for medically unnecessary power wheelchairs that were sometimes never even delivered.

In one case, Akinola Afolabi, 53, of Long Beach, owner of Emmanuel Medical Supply, allegedly submitted more than $2.6 million in in false and fraudulent billing to Medicare.

In another case, Queen Anieze-Smith, 52, of Encino, and Abdul King- Garba, 47, of Westwood, owner-operators of ITC Medical Supply, allegedly submitted more than $1.8 million in false and fraudulent billing to Medicare, according to court documents.

In the third case, Clement Etim Aghedo, 53, of Fontana, owner of Ace Medical Supply Co., allegedly submitted more than $1.8 in false and fraudulent claims to Medicare.

The charge of health care fraud carries a potential maximum penalty of 10 years in federal prison; money laundering carries a potential 20 years in prison; and aggravated identity theft carries a mandatory two-year prison term.

Stocks continued their climb into uncharted territory on Friday, racking up the fourth week in a row of gains as encouraging economic data prompted investors to buy shares of growth companies.

The Dow Jones industrial average and the Standard & Poor’s 500-stock index finished at highs, driven by gains in energy and industrial shares. The indexes have pushed to a series of high levels as part of the rally that has lifted equities more than 16 percent for the year so far.

The Dow Jones industrial average gained 121.18 points, or 0.80 percent, to close at a milestone 15,354.40. The S.& P. 500-stock index rose 17 points, or 1.03 percent, to end at a record 1,667.42.

The Nasdaq composite index climbed 33.73 points, or 0.97 percent, to finish at 3,498.97 — its highest close since October 2000.

In a sign of how far the market has come, the S.& P. 500 is about 1,000 points above the low it hit in March 2009 in the wake of the credit crisis and recession.

“It’s hard to hold this market down,” said Michael Sheldon, chief market strategist at RDM Financial in Westport, Conn.

Data released on Friday showed Americans felt better about their economic and financial prospects in early May, with consumer sentiment at its highest in nearly six years, while a gauge of future economic activity rose in April to a near five-year high.

“If you believe the economy is going to gradually get better and that global growth will improve, the parts of the market that have not benefited so far, like cyclicals, will probably be the next group to outperform,” Mr. Sheldon said. Cyclical industries are those that do well when times are good, like an airline that benefits from more people flying on vacation.

The S.& P. energy sector index gained 0.8 percent, with Exxon Mobil up 1.2 percent at $91.76.

Boeing shares led the S.& P. 500’s industrial sector index higher with a 2.4 percent advance to $98.92, its highest since October 2007. The S.& P. industrial index rose 1.4 percent.

The rate of growth in the economy has been expected to slow in the second quarter as tighter fiscal policy started to take effect. But recent improvements, including in the labor market and retail sales, suggest the recovery remains resilient.

“We are still recovering,” said Doreen Mogavero, chief of Mogavero, Lee & Company in New York, who also noted that the comeback was slow. She added that markets in the United States, for all their troubles, were “still the best place to be at this moment.”

Earlier in Friday’s session, the Dow touched a high at 15,357.40. For the week, the Dow advanced 1.7 percent, while the S.& P. 500 climbed 2.1 percent and the Nasdaq rose 1.9 percent.

J. P. Morgan raised its year-end target for the S.& P. 500 to 1,715 from 1,580, implying a gain of just under 3.5 percent for the index for the rest of the year.

“We realize investors are apprehensive about making fresh money purchases, but we see the risk/reward as particularly attractive in technology, health care and financials,” said the client note from Thomas Lee, J. P. Morgan’s United States equity strategist.

J. C. Penney shares lost 4.2 percent to $18.01 after the retailer reported another steep quarterly loss on weak sales and heavy clearance deals, and the chief executive, Myron Ullman, cautioned that he needed time to fix the company’s problems.

Tableau Software, a maker of data analysis software, surged in its first day of trading as investors bet the rising interest in Big Data would drive its growth. Tableau surged 64 percent to $50.75.

The price of the benchmark 10-year Treasury note fell 22/32 to 98 5/32, increasing the yield to 1.95, from 1.88 on Thursday.

One year later, Facebook has figured out mobile, experts say.

The social networking giant may have had a rough patch after its IPO, but Facebook has quickly turned things around by cashing in on its users' obsession with mobile.

"One of the big complaints at the company's IPO was that it doesn't make any money on mobile, and that was true and fair at the time," said Jason Stein, founder and president of the social media agency Laundry Service.

Facebook had zero revenue on its mobile-ad platform when it went public but posted $375 million on mobile in the first quarter, Stein said.

"It's amazing that a company was criticized for lacking any mobile presence was able to completely build a new app and monetize it as quickly as it was able to build it," Stein said.

Almost 70 percent of Facebook users are on the network's mobile platform, and about 30 percent of all ad revenue is generated via mobile—advertisers like that because they see huge growth opportunity, Stein said.

Eden Zoller, principal consumer analyst at Ovum, said its crucial that Facebook continue to expand into emerging markets.

"I think [ad revenue] will increase, and there's a real imperative for Facebook to continue that momentum," Zoller said. "More people are interacting with people on mobile phones, some of them on an exclusive basis. ... Facebook needs to ensure that the advertising dollars go with that migration."

Clearly, Facebook hasn't always had the right mobile approach.

CEO Mark Zuckerberg admitted in September that the company's mobile strategy had been too focused on HTML5 (the programming language that's compatible with any browser and doesn't require add-ons to function) and not enough on native apps, which users preferred.

"It completely blew up in their face, but they didn't waste any time," said Krishna Subramanian, chief marketing officer of Velti, a mobile marketer. "They got to work building very high-performing applications. They bought Instagram. They focused on mobile growth. The key thing that Zuckerberg does well is identify when things don't work and change quickly."

Home isn't a revenue producer yet but definitely has potential, according to Stein. Home may appeal to marketers because odds of engagement with an ad increase dramatically if Facebook is always running on a user's phone.

But Home won't be a big earner for a while, Subramanian said.

"It will continue to evolve," he said. "Zuckerberg's gut feelings are very spot-on, but if you make 100 changes, not every one is going to work."

Facebook also partnered with manufacturer HTC on a smartphone, HTC First, preloaded with Home, and Subramanian said he could see the company taking on more hardware partnerships.

"You don't just go out and build a phone—you focus on what your core strengths are," he said. "I could see them building an OS to power devices or deeper integrations" on platforms.

"I definitely think things are going in the right direction," Subramanian said. "I'm definitely excited about what they are doing from a mobile-ad perspective and excited to see how they drive the connected consumer."

STOCKHOLM, Sweden, May 17 (UPI) -- In utero exposure to depression medications may increase the risk of autism spectrum disorders in less than 1 percent of cases, researchers in Sweden say.

First author Dheeraj Rai, a clinical lecturer at the Department of Public Health Sciences at Karolinska Institutet in Stockholm, and colleagues at the University of Bristol; Avon and Wiltshire Partnership Mental Health NHS Trust in Bristol, England; and Drexel University School of Public Health on Philadelphia said the study involved 4,429 cases of autism spectrum disorder -- 1,828 with and 2,601 without intellectual disability and 43,277 age and sex matched controls.

The study involved 1,679 cases of autism spectrum disorder and 16,845 controls with data on maternal anti-depressant use.

The study, published in the British Medical Journal, said parental depression and other characteristics were recorded in administrative registers before the birth of the child. Maternal anti-depressant use, recorded at the first antenatal interview, was available for children born from 1995 onwards.

A history of maternal -- but not paternal -- depression was associated with an increased risk of autism spectrum disorders in offspring, the study said. In the subsample with available data on drugs, this association was confined to women reporting anti-depressant use during pregnancy irrespective of whether selective serotonin reuptake inhibitors or non-selective monoamine reuptake inhibitors were reported, the study said.

"Whether this association is causal or reflects the risk of autism with severe depression during pregnancy requires further research," the study authors wrote in the study. "However, assuming causality, anti-depressant use during pregnancy is unlikely to have contributed significantly towards the dramatic increase in observed prevalence of autism spectrum disorders as it explained less than 1 percent of cases."
